Select your campus (We are Albuquerque).

Select your building (We are 253 Biomedical Research Facility).

Fill in all other information on the next page (We will use area 2 maintenance for any kind of electrical and plumbing needs and select custodial for any kind of paper towel and trash needs) and hit submit. You may leave the index number blank.

Keep track of the request number that they give you to reference later.
